Public Encouraged to Complete Shelby County Health Needs Survey

Myrtue Medical Center, Myrtue’s Public Health, along with the Shelby County Board of Health, and other community providers, need your help in learning more about the specific health needs of Shelby County residents. This information will be used to develop plans for future health improvements to the community.

Myrtue Medical Center and Myrtue’s Public Health Department—in partnership with community organizations, schools, and businesses—use this feedback to prioritize plans for increasing awareness, expanding wellness services, and improving community engagement.

The 2026 Community Health Needs Assessment survey is now available at myrtuemedical.org. To participate, click on 'Patients + Visitors' and then select 'Community Health Needs Assessment.' The survey closes on Friday, April 3, 2026. Your responses will be used to develop a formal strategic action plan during the Community Health Needs Forum in June.

If you do not have internet access or prefer not to use the online version, paper surveys are available. You may pick one up at the Public Health office (2712 12th St.) or request to have one mailed to you by calling (712) 755-4308.

The progress report for the most recent assessment is also available at myrtuemedical.org. Navigate to ‘Patients + Visitors’ → ‘Community Health Needs Assessment’ and scroll to the bottom of the page to find the ‘2023-2026 Health Improvement Plan Progress Report.’

These results will guide us in identifying and prioritizing the most pressing health needs while addressing gaps in care.”
