Community Health Needs Assessment Forum

You are invited to participate in a community forum to discuss the health needs of Shelby County.
Your participation is critical to assist Myrtue Medical Center in identifying and developing action steps to address the healthcare needs of our community. We value your ideas and suggestions. Open to all community members.

The forum will abide by the CDC recommendations for safe gatherings. We will practice social distancing, 50% room capacity, and participants are encouraged to wear a mask or cloth face covering.

Promoting Population Health

Myrtue Medical Center is committed to promoting population health among the communities we serve. The following Community Health Improvement Plan is a comprehensive approach to assessing Shelby County’s health and developing an action plan for improvements. A Community Health Improvement plan has two parts: a Community Health Needs Assessment (CHNA) and a Health Improvement Plan (HIP).

External Data was collected through surveys and a community forum to establish ideas on current Shelby County health needs and ways for Myrtue Medical Center to meet them. Various representatives of community organizations, school and business representatives and community residents were involved in the Community Health Forum. Internal data was collected through multiple hospital meetings involving employees, Med Staff, Board of Trustees and Senior Administration.

Myrtue Medical Center has prioritized the following strategic issues in order to improve health and wellness services within the communities we serve.

  • Mental Health: Increase access to Mental Health services such as support groups, suicide prevention and substance abuse programs in Shelby County.
  • Community Outreach: Increase the communities’ awareness of MMC health services and benefits by participating in events benefiting our patient population and engaging the community as partners in health education.
  • Marketing: Formalize a comprehensive plan to address all aspects including messaging, branding, marketing, graphic standards, and advertising to highlight and promote MMC services and providers.
  • Wellness Education: Engage the community, individuals, organizations, and businesses to promote healthy lifestyles, which include increased physical activity, integrated primary care and mental health services and expanded education for chronic disease management.
  • Internal Planning: Expand services and develop a master facility plan to promote excellent service to the community and employees.
  • Enhanced Services: Evaluate, update, create, and expand services to address the demands identified in the community health needs assessment.
