Athletic Physical Schedule

School physicals are again being provided for students entering 7th-12th grades by Myrtue Medical Center Clinics.  Click here for schedule.  Payment is due at the time of service; no insurance will be submitted.  Print the physical form below.  Prior to the appointment, parents are to complete and sign the physical form.  Both the physical form and payment must be presented at the appointment.

Myrtue Medical Center will also be offering immunizations during athletic physical appointments for Tdap, Meningococcal, and/or Hepatitis B.  Parental consent is needed.  For more info on any of the vaccines, click below.

HCCMS Family Planning Clinics

Public Health will be holding Family Planning Clinics on Tuesdays in the Community Health Department on the following dates and times:

July 22 - 8:00 am to 12:00 noon

July 29 - 12:00 noon to 7:00 pm

August 5 - 8:00 am to 12:00 noon

August 19 - 11:00 am to 7:00 pm

August 26 - 8:00 am to 12:00 noon

September 2 - 11:00 am to 7:00 pm

September 9 - 8:00 am to 12:00 noon

September 23 - 11:00 am to 7:00 pm

September 30 - 8:00 am to 12:00 noon

July 4th, 2006 is the date Myrtue Medical Center became tobacco free. We are currently educating staff about the long-term effects of tobacco use, and how to deal with those around you who continue to smoke. Smoke Free
