2010 Athletic Physicals

School physicals are again being provided at Myrtue Medical Center Clinics for students going into 7th-12th grades.  Payment is due at the time of service; no insurance will be submitted.

• For established patients $15
• For new patients $25
• At any other time (other than dates/times listed below) $25
